Providence Farm Community (“Providence Farm,” “we,” “us” or “our”) respects the privacy of everyone who visits pfcommunity.org. This Privacy Policy explains what information we collect through this website, how we use and protect it, and the choices you have. By using this website you agree to the practices described here.
This policy covers information collected through this website and through the calls, texts, emails and forms you send us from it. It does not govern clinical records. Health information created or kept in the course of treatment is protected under the laws described below and our Notice of Privacy Practices, which is given to every client and is available on request.
Information you give us. When you complete a form, call, text or email us, we receive your name, phone number, email address and anything you choose to tell us, which may include information about yourself or a loved one, such as substance use, mental health or insurance details.
Information collected automatically. Like most websites, we collect limited technical information when you visit, such as IP address, browser and device type, pages viewed, time on page and the site that referred you. We use this in aggregate to understand how the site is used.
As a behavioral health and substance use treatment provider, we treat the information you share with us with care. Health information connected to treatment is protected by the Health Insurance Portability and Accountability Act (HIPAA), federal confidentiality rules for substance use disorder records (42 CFR Part 2), and the Illinois Mental Health and Developmental Disabilities Confidentiality Act (740 ILCS 110). We do not disclose that information without your written consent except as those laws allow, such as in a medical emergency or where disclosure is required by law.
Please keep in mind that a website form is not a clinical record. If you would rather not put sensitive details in writing, call us at (847) 409-6230.
With your permission, we use Google Analytics and HubSpot on some general information pages of pfcommunity.org and iop.pfcommunity.org (our Community IOP site) to count visits and taps on our phone number links and see which pages are useful. These optional tools are off until you turn them on with “Analytics settings” in the “Data & privacy” line at the bottom of those pages, where you can also change your choice at any time. Nothing pops up and nothing is switched on for you. Each website has its own settings and remembers your choice in this browser for up to 12 months.
Google Analytics and HubSpot are separate choices. If you previously allowed Google Analytics, that choice still covers Google Analytics only; HubSpot stays off until you allow it. HubSpot records which pages of our site you view. If you allow HubSpot and later submit one of our forms, HubSpot may connect those earlier page views to your inquiry. HubSpot also automatically records clicks on links and buttons (the element’s text and link address, never what you type), and recognises your browser on that website with a random visitor ID kept in its cookies and a browser fingerprint value. HubSpot’s cookies (such as __hstc and hubspotutk) are set separately for each of our websites and are not shared between them.
When a service is allowed, your browser sends it your IP address, browser and device details, its cookies, the page address and title, and the page you came from. These tools never run on pages whose address contains search or query details. Google Analytics is given only the address of the website you came from, not the full page. HubSpot reads the referring page from your browser itself, so we do not load it at all when that referring address includes a page path or query details; between our own pages your browser is told to send only our website address. They do not run on our contact, donation or privacy pages. We do not send names, messages or other form entries through this analytics code. Our contact forms are processed separately by HubSpot, and donations separately by Stripe, regardless of your analytics choice. Advertising storage, personalization and advertising features stay off.
Turning a service off stops it on this browser for that website and removes its cookies there; it does not delete information already sent. Each website keeps its own choice. Before you choose, the pages that offer these settings make no Google Analytics or HubSpot analytics requests; like every page on this site, they still load fonts from Google Fonts and may load other third-party content, such as linked or embedded video. You can also block or delete cookies in your browser settings. Some browsers send “Do Not Track” signals. Because there is no common standard for them, this site does not respond to those signals.
Email and standard text messages are not fully secure. Please avoid sending detailed clinical information that way. If you text us or agree to receive texts, message and data rates may apply. You can reply STOP at any time to opt out. We do not sell or share mobile numbers or text consent with third parties for marketing.
We do not sell, rent or trade personal information. We share it only:
We do not collect biometric identifiers or biometric information, such as fingerprints, face geometry or voiceprints, through this website, as those terms are defined in the Illinois Biometric Information Privacy Act (740 ILCS 14).
We use reasonable administrative, technical and physical safeguards to protect personal information. No method of transmission over the internet is completely secure. If a breach affecting your personal information occurs, we will notify you as required by the Illinois Personal Information Protection Act (815 ILCS 530) and other applicable law.
We keep inquiry information only as long as needed to respond to you and for our legitimate operational, legal and accreditation purposes. Clinical records are kept as required by Illinois and federal law.
You may ask us what personal information we hold about you from this website, ask us to correct or delete it, or ask us to stop contacting you, at any time. Email [email protected] or call (847) 409-6230. Your rights regarding clinical records are described in our Notice of Privacy Practices.
Our programs serve adults. This website is not directed to children under 13, and we do not knowingly collect personal information from them. If you believe a child has sent us information, contact us and we will delete it.
This website links to other sites, including our Community IOP site and the secure third-party processor that handles online donations. Those sites have their own privacy practices, and we are not responsible for them.
We may update this policy from time to time. The date at the top of this page shows when it was last revised. Changes take effect when posted.
